Welcome to the myUSAi online information center for FREE official forms of the U.S. State Department. Typically these forms are submitted or filed for processing with a U.S. Consulate office abroad. We also include FREE forms for use by U.S. Citizens living or travelling abroad.
Warning! Other websites may offer State Department forms. Some will sell you a downloadable form for a fee. These sites are not affiliated with the U.S. government, and these sites may not have the latest versions of forms. In some circumstances, use of older forms will result in your application or petition being denied or delayed.
The latest versions are always available here. This page was last updated 09/23/09.

The new DS-160 Online, Nonimmigrant Visa Electronic Application, will eventually replace current nonimmigrant application forms DS-156, 157, 158, and other related forms, such as the DS-156E and 156K. The roll-out of the new DS-160 will be gradual. Therefore, not all U.S. Embassies and Consulates will be required to use the form at the same time. Learn More about the form DS-160 and review the list of U.S. Embassies and Consulates currently using the new form DS-160.
These electronic forms (except DS-156, DS-1648, and DS-4024) require Adobe Acrobat 5.0 or higher. |
